Okay, I'll be honest and hope everyone else is too.
I have seeral bad cooking stories: Once I put a ramen noodle container in the microwave but forgot to add water.
Another time I was baking cookies and I decided last minute to add this chocolate flavoring to teh dough, and it turned them a horrible brownish green color; nobody would eat them.
When I was a little kid, I put a plastic tray in the oven and it started to melt.
Also when I was a kid, I tried to make "fruit soup" by cutting up fruit, putting water in the bowl, and microwaving the whole thing. Then I made my family eat it.
;D

lol Just blame it all on childhood right? I know what I did and I'm sure everyone else has...I burnt toast. Yeah, whatever. I've also burnt pancakes and the really easy stuff. But that was when I was being stupid and not paying attention. The fire alarm went off a few times and my dog got scared and went under the bed. I hate cooking accidents b/c it makes me feel so bad. lol I can tell you a mistake someone else made and that was putting too much baking soda in the cookies. He managed to spill the bag. They were still made anyway and it tasted like....baking soda. LOL

I always go wrong when trying to make substitutions... you know, like yogurt instead of sour cream, or oil instead of butter. Some people can get away with it, but not me.

I used to take things i hated out of recipes, but forget to add substitutions. And I wondered why the recipes never came out just right
